Patient Resources

Dade Physical Therapy welcomes new patients. Here are a few items to note for your first visit. Please be prepared with the following items:

• Prescription and Authorization from your physician, unless it has already been faxed to our office
• Driver’s License or Photo ID
• Insurance Card

Please arrive approximately fifteen minutes early to the first appointment so that your paperwork can be completed. Please wear loose, comfortable clothing so that we will be able to properly evaluate your current condition. Athletic or closed-toe shoes should also be worn.
